HOME CARE PACKAGE PHYSIOTHERAPY
OUR SERVICES
At NeuCore Physiotherapy, we believe rehabilitation should support people to stay independent in the place they feel most comfortable — their home. We work with Home Care Package participants to provide personalised physiotherapy that focuses on improving mobility, building strength, and helping people continue doing the everyday things that matter to them.
LOOKING TO HAVE PHYSIOTHERAPY IN THE HOME? HERE'S HOW TO GET STARTED.
1. Eligibility for in-home support: an assessment is completed by the Aged Care Assessment Team (ACAT). During this visit, they look at your age, health, mobility, and the types of support that may help you remain safe and independent at home.

2. Approval for home support funding: If you are eligible, funding will be approved through the Home Care Packages program. Once approved, you can choose a provider who will help organise and manage the services included in your care plan.

3. Organising the services you need: Your chosen provider will work with you to create a plan that supports your daily living and health goals. This may include a range of services such as personal care, domestic support, and allied health services like physiotherapy.

4. NeuCore Physiotherapy services delivered at your home: After the provider confirms physiotherapy as part of your support plan, NeuCore physiotherapy can visit you at home. We focus on improving mobility, strength, and confidence while helping you work towards meaningful goals that support your independence.
